Format guide
There are now three oral GLP-1 medicines, and two different daily rules.
Three of them are now approved as tablets. Two rules divide them: oral semaglutide needs an empty stomach and a timed wait, and orforglipron does not. That difference shapes your day more than anything else on the label.

What exists
The oral options, and what each is approved for
This is the part most articles get wrong, so it is worth stating precisely from the labels.
Oral semaglutide — Rybelsus and Ozempic tablets
Both share one FDA label. They are indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes, and to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes at high risk for them. The label also states the two are not substitutable on a milligram-to-milligram basis.
Oral semaglutide — Wegovy tablets
Same molecule, different label and a different job. Wegovy tablets are indicated for cardiovascular risk reduction and for weight reduction in adults. They start at 1.5 mg once daily for 30 days and titrate every 30 days to a maintenance dose of 25 mg once daily — a very different set of numbers from the diabetes tablets, which is exactly why the label warns the products are not interchangeable milligram for milligram.
Orforglipron — Foundayo
Approved and marketed as Foundayo, indicated in combination with a reduced-calorie diet and increased physical activity to reduce excess body weight and maintain weight reduction long term, in adults with obesity or adults with overweight who have at least one weight-related condition. It is a small-molecule GLP-1 receptor agonist rather than a peptide.
If you have read that orforglipron is still in development, that was true until recently. The current US prescribing information for Foundayo is dated 29 July 2026.
The daily difference
A timed fasting window, or no window at all
Oral semaglutide's label instructs that it is taken orally once daily on an empty stomach in the morning with water — up to four ounces — and not with other liquids. Foundayo's label says simply: take it orally once daily, with or without food.
Follow whichever set of instructions your own prescription carries; that is on your label and your pharmacist can confirm it. What matters for choosing a format is that these two build very different mornings.

Why the window exists
Absorption, in the label's own numbers
Semaglutide is a peptide, and peptides are poorly absorbed from the gut. The label puts the estimated absolute bioavailability at 0.4% to 1% for Rybelsus at 3, 7 and 14 mg, and 1% to 2% for Ozempic tablets at 1.5, 4 and 9 mg. Absorption occurs predominantly in the stomach, and maximum concentration is reached about an hour after the dose.
When that little is absorbed under ideal conditions, the conditions matter. That is the whole reason for the empty stomach, the small volume of plain water and the wait. Orforglipron, being a small molecule, does not carry that constraint — which is the practical advantage of its format.
Side effects by format
The rates are not interchangeable between orals
A lot of writing about “oral GLP-1 side effects” quotes one range for all of them. The labels do not support that: the two oral medications were studied in different populations at different doses, and their nausea rates are far apart.

Read down the population column before comparing any two rows. A diabetes trial and an obesity trial are different groups of people at different doses, and that accounts for much of the spread.
Discontinuation is measured too: on Foundayo, 3% to 6% of patients stopped because of gastrointestinal reactions depending on dose, against 0.7% on placebo.

The routine

Oral GLP-1 questions
Is Rybelsus approved for weight loss?
No. The shared US label for Rybelsus and Ozempic tablets indicates them as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes, and to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes at high risk. The oral semaglutide that does carry a weight-management indication is a different product, Wegovy tablets; the other oral GLP-1 approved for weight reduction is orforglipron, marketed as Foundayo.
What is the difference between Rybelsus and Wegovy tablets?
Both are oral semaglutide and both are taken on an empty stomach with a 30-minute wait, but they are approved for different things at very different doses. Rybelsus is for glycemic control in type 2 diabetes at 3, 7 and 14 mg. Wegovy tablets are for cardiovascular risk reduction and weight reduction, starting at 1.5 mg once daily and titrating every 30 days to a 25 mg maintenance dose. Should orforglipron be taken with or without food?
Foundayo's US prescribing information states that it is taken orally once daily, with or without food, and that tablets are swallowed whole. That is the label's own wording. Follow the instructions on your own prescription, and ask your pharmacist if anything on it differs from what you have read.
What is the fasting window for oral semaglutide?
The label instructs that Rybelsus and Ozempic tablets are taken once daily on an empty stomach in the morning with water, up to four ounces, and not with other liquids. The reason is absorption: the label puts estimated absolute bioavailability at 0.4% to 1% for Rybelsus and 1% to 2% for Ozempic tablets, absorbed predominantly in the stomach.
Do oral GLP-1s have fewer side effects than injections?
The labels do not support a single answer, because the rates differ more between the two oral medications than between oral and injectable. Oral semaglutide reported nausea in 11% at 7 mg and 20% at 14 mg against 6% on placebo in type 2 diabetes trials; Foundayo reported 26% to 35% against 10% in obesity trials; injectable Wegovy 2.4 mg reported 44% against 16%. The populations and doses differ, so compare rows with care.
Are Rybelsus and Ozempic tablets the same thing?
They share one FDA label as oral semaglutide, but the label states explicitly that they are not substitutable on a milligram-to-milligram basis, and their approved strengths differ: 3, 7 and 14 mg for Rybelsus and 1.5, 4 and 9 mg for Ozempic tablets.
